Our In-Home Services provide person-centered, compassionate and reliable in home care for you or your loved one. Our dedicated team of Registered Nurses and Certified Nursing Assistants will work with your family to develop a personal plan of care that addresses your individual and unique needs such as:

Programs To Meet Your Needs
Services are focused on meeting the needs of seniors and adults with disabilities with an emphasis on maintaining their independence and enhancing their overall quality of life. Home care services allow recipients to remain safely in their own homes, avoiding nursing home placement. In-home care includes assistance with personal care (bathing, dressing & grooming), household tasks (laundry & cleaning), shopping and errands, meal preparation, and respite care for the family. These services also seek to address the social and emotional needs of the individual in an effort to improve both mental and physical health.

Private Pay Services
For people who need assistance in their home, but are not eligible under other funding programs, we offer private pay services at a very competitive hourly rate. You will get the services and care you need, with the added security of knowing you have a trained and certified nursing assistant who has been through a thorough screening process, available to help in your home.

Home and Community Care Block Grant (HCCBG)
Provides in-home aides to assist with the personal care needs of frail or disabled people age 60 years or older who wish to continue living as independently as possible or have caregivers in need of respite from providing heavy care. Personal Care tasks include assistance with bathing, dressing, walking, transfers and meals.

HCCBG services have no financial eligibility requirements; however service recipients may make donations to offset some of the cost of their care. The number of in-home aide hours allocated to each person is determined by the person’s medical condition, the availability of other caregivers, and the amount of funding designated for the program. Shortages in funding mean that applicants will frequently be placed on a waiting list. Applicants will be screened and prioritized according to state requirements.

Personal Care Services
To qualify for Personal Care Services (PCS) your Primary Care Physician will need to complete the "PCS Request for Assessment/Attestation Form" and send to Liberty Health Care the Independent Assessment Entity designated by Community Care North Carolina (CCNC). Liberty Healthcare completes the screening and assessment for this program and determines eligibility and parameters of service based on your individual needs. You may receive assistance with the following under PCS: bathing, dressing, personal hygiene, grooming, mobility, transferring, medication reminders, toileting and continence care. Once you have completed your referral and been approved for services you may choose us as your Personal Care Services provider!

Companion Care
This is non-medical, non-hands on supportive in home care. Companion Assistants, trained, screened, and caring staff  can provide a wide range of services which includes meal planning/preparation, light housekeeping, transportation, respite care for families, companionship, medication reminders and more. You will work directly with our Companion Care Coordinator to develop a plan of care that meets your needs.

Sample packages include:
• Health Appointments -Transportation and attentive support during health appointments
• Rise and Shine - Assistance with morning routines
• Turn Down & Tuck In - Assistance at the end of the day
