In-Home Services
Private Pay Services
For people who need assistance in their home, but are not eligible under other programs, we offer private pay services at a very competitive hourly rate. You will get the services you need with the security of knowing you have a trained and certified nusring assistant who has been through a thorough background screening process.
Home Care Block Grant
Home and Community Care Block Grant (HCBG) provides in-home aides to assist with the personal care needs of frail or disabled people age 60 years or older who wish to continue living as independently as possible or have caregivers in need of respite from providing heavy care.
Personal care tasks include assistance with:
- Bathing
- Dressing
- Walking
- Transfers
- Meals
HCBG services have no financial eligibility requirement; however, service recipients may make donations to offset some of the cost of their care.
The number of in-home aide hours allocated to each person is determined by the person’s medical condition, the availability of other caregivers, and the amount of funding designated for the program. Shortages in funding mean that applicants will frequently be placed on a waiting list. Applicants will be screened and prioritized according to state requirements. There is currently a waiting list for these services.
Medicaid Services
CAP/DA
Community Alternatives Program for Disabled Adults (CAP/DA) is a Medicaid program designed to give disabled adults (age 18 and over) who are at risk of unwanted nursing facility placement the option of remaining safely at home with a package of services to complement their current family support. CAP/DA provides in-home aides to assist clients with personal care tasks, such as:
- Bathing
- Dressing
- Walking
- Toileting
- Incontinence care
- Meals
In addition to personal care assistance, in-home aides may also perform light housekeeping tasks and help with shopping and errands.
Other CAP/DA services:
- Case management
- Telephone Alert Service
- Home Mobility Aids(wheelchair ramps, doorway widening, safety railings, non-skid surfaces, handheld showers, and grab bars)
- Nutritional supplements, e.g., Ensure, Boost
- Incontinence supplies
- Respite care
Referrals must be approved at intermediate or skilled level of care and be eligible for Medicaid in order for the case management team to conduct a CAP/DA assessment. The assessment must indicate that the client is appropriate for participation in the program before a plan of care can be developed, approved and implemented.
Each county is allotted a certain number of CAP/DA slots. When no slots are open, the referral will be placed on a waiting list and processed a soon as possible.
